Physics Account Request

You must be affiliated with Harvard University, be attached to a Harvard Physics internal department or faculty
member and have a valid Harvard ID to request an account with Harvard Physics. If you do not meet these criteria
in your answers below, your request will be discarded.

NOTE: Graduate students should not use this form. Please contact the graduate programs administrator if you have
questions about getting an account.
(Exception: Applied Physics students from SEAS, etc. may use this form to request a Physics
account as they often need to use our library terminals)

as this is also where their password sheet will be delivered.

E-Mail - For security reasons, we can only e-mail the sheet to a valid Physics account or faculty e-mail.
Interoffice - For security reasons, we can only deliver the sheet to a valid Physics staff or faculty member.
Pick Up - When complete, we will inform you where you can pick up your password sheet. Harvard ID required.
PLEASE NOTE: We cannot e-mail or mail the account information directly to the requester.
