Physics Account Request

You must be affiliated with Harvard University, be attached to a Harvard Physics internal department or faculty
member and have a valid Harvard ID to request an account with Harvard Physics. If you do not meet these criteria
in your answers below, your request will be discarded.

NOTE: Graduate students should not use this form. Please contact the graduate programs administrator if you have
questions about getting an account. (Exception: Applied Physics students from SEAS, etc. may use this form to request a Physics
account as they often need to use our library terminals)

Faculty members, should simply enter their or their administrative assistant's name
as this is also where their password sheet will be delivered.

E-Mail - For security reasons, we can only e-mail the sheet to a valid Physics account or faculty e-mail.
Interoffice - For security reasons, we can only deliver the sheet to a valid Physics staff or faculty member.
Pick Up - When complete, we will inform you where you can pick up your password sheet. Harvard ID required.
PLEASE NOTE: We cannot e-mail or mail the account information directly to the requester.
It must be sent to the sponsor or picked up by the requester accompanied by an valid Harvard ID.
